Rose Cottage Volunteer Application Form
All Fields Must Be Completed.
All volunteers are required to provide an Ontario Driver’s License (for visiting volunteers), reliable transportation, and valid auto insurance. Volunteers must also obtain a Vulnerable Police Clearance, which will be completed through Rose Cottage. In addition, volunteers must have access to a computer and the internet to support communication and program requirements.
